This one beats me. The only think I can think of is I somehow spawned where our flag bearer had been before dying but I didn't see him anywhere and nobody friendly was left. Two enemy flag bearers were close by though.
I skipped through part of the dead scene because I was steam chatting but yes, it skipped straight from 12 seconds to spawning.
